Review and sign your services agreement.

This is the working agreement between your firm and Velocity: what we do, what it costs, and how your records are protected. Version 1.0, signed electronically.

Velocity Retrievals Services Agreement

This agreement is between your firm (“Firm”) and Velocity Record Retrievals (“Velocity”).

1. Services

Velocity retrieves medical records, bills, and related documents from healthcare providers and other custodians on the Firm’s behalf, acting as the Firm’s designated records-retrieval agent. Velocity’s AI agents perform outreach and follow-up; trained staff review every outbound communication.

2. Pricing

No subscriptions and no card on file. The Firm is invoiced per delivered request at the published flat rate, plus provider-imposed fees passed through at cost. Payment terms are net 30, ACH preferred.

3. Records protection

All records are transmitted and stored encrypted, access is restricted to the Firm’s workspace, and Velocity maintains administrative, physical, and technical safeguards consistent with HIPAA. Records are never used for any purpose other than fulfilling the Firm’s requests.

4. Term

Either party may end this agreement at any time with written notice. Requests in flight at termination are completed and invoiced normally.

Authorize Velocity with providers.

Providers need one letter from your firm naming Velocity as your records-retrieval agent. Sign our standard letter electronically, or print it on your letterhead and upload the signed copy.
